So I sold a rather expensive shirt for $100+
Buyer opens a return, says it doesn’t fit
He is asking if I can exchange it for a size small which I have in stockHere’s how the request looks:

Under reason for return at the bottom of the page buyer put doesn’t fit
It seems eBay is clear that I don’t have to take the return but I don’t want this to turn into a INAD and then be forced to take it. As it stands now, buyer would have to pay return shipping
I’m also worried that the buyer could be scamming me and end up returning a completely different item. The buyer has zero feedback, but I find that it common when I’m selling streetwear items. Also the item was new with tags, if the buyer took the tags off and wants to return the value of the item significantly decreases
What would you guys do?

Since I take returns on most of my items, I wouldn’t have an issue with the exchange.
You could send him a message asking whether the tags were still attached and the item unworn. If he says yes, then perhaps you should consider taking the return. He will have to pay the shipping for the replacement as well. I think he will just have to outright purchase the new one after you refund him for the old one.
If you refuse the return, I don’t know that he could do an INAD. I’ve never been in the situation, but I think you could call eBay and explain that he opened a normal return first for fit.
However, he could give you negative feedback if you refuse to accept the return. So, I recommend accepting it.

I wouldn’t have an issue doing an exchange, either. If they send back the item and it’s not in the original condition, I would call ebay at that point to figure out how to proceed from there. No reason worrying about a problem that hasn’t happened yet. But I have no experience doing exchanges, so I don’t know how to actually “do” an exchange on the ebay system.
Where are you getting the information that “ebay is clear that you don’t have to accept the return”? My experience has been that if you don’t accept the return (INAD or not), ebay will accept it for you in several days.
Don’t worry about it turning into an INAD – there is no way to change the reason of a return.
Personally, I see it as good news whenever I get a return and the buyer said fit was the reason – that means it’s NOT an INAD, and I don’t have to pay for return shipping.

Yeah, accept the exchange. He has to return the item first before you send the smaller shirt.
It’s really interesting that the fear of buyers purposefully sending back different items is so strong. I’ve only heard from one seller here where this has happened.
In the thousands and thousands of items we’ve sold, it’s happened once to us. Lady was a drunk and very confused about her life. eBay covered us.

How are you supposed to do an exchange on eBay?
Do I just accept the return and have the buyer purchase the other shirt? -

I did an exchange once. I accidentally sent the wrong item. If I remember correctly, when the buyer opened a case, there was an option for me to select an exchange. I paid for the return shipping since it was my fault. Ebay tracked the returned shipping and then tracked my shipment of the exchanged item.

So buyer sent me this message today:
“Hello I get sent back the shirt back today but it didn’t give me a tracking number”
The case on eBay still says “Waiting for buyer to ship item”. When returning an item is the seller required to go through eBay and print a label on their website? I’m not sure if they just went and purchased shipping at their local post office. Should I reply to the buyer?

Buyers aren’t required to get a label through eBay, although the cost is lower than going through the post office and tracking is automatically uploaded. It’s also in their best interest to get a tracking number to protect themselves. If you never get their package, they can’t prove that they sent it out.
You don’t have to respond. If you want, you can say “I’ll be on the lookout for it”, or something to that effect.
